UIBC blood test: what unsaturated iron binding capacity means
UIBC is the amount of iron-carrying capacity in your blood that is empty. Iron travels on a protein called transferrin; UIBC measures the transferrin seats with no iron in them. A high UIBC usually means the body is short of iron. A low UIBC usually means the seats are mostly full — or that there is less transferrin than normal.
How UIBC fits with the rest of the iron panel
UIBC is rarely ordered on its own. It appears on an iron panel next to serum iron, TIBC and transferrin saturation, and the four are tied together by simple arithmetic:
- TIBC (total iron binding capacity) = serum iron + UIBC. All the seats, full and empty.
- Transferrin saturation = serum iron ÷ TIBC × 100. The share of seats that are full.
- UIBC = TIBC − serum iron. The empty seats.
Because of this, UIBC adds little information that TIBC and saturation don’t already carry. Many labs measure UIBC directly and calculate TIBC from it, which is why it shows up on your report at all.
A worked example
Suppose your report shows serum iron of 40 µg/dL and TIBC of 450 µg/dL.
- UIBC = 450 − 40 = 410 µg/dL — most of the capacity is empty.
- Saturation = 40 ÷ 450 × 100 = 8.9% — well under the ~20% level clinicians watch for.
High UIBC, high TIBC and low saturation together are the classic signature of iron deficiency — especially if ferritin is also low. On its own, though, none of these numbers is a diagnosis.
What a high UIBC can mean
- Iron deficiency — by far the most common reason. Less iron, and the body makes more transferrin to find it.
- Pregnancy, particularly later pregnancy, raises transferrin.
- Estrogen — the combined contraceptive pill and hormone therapy can raise transferrin.
What a low UIBC can mean
- Iron overload — the seats are low because they are full. Saturation will be high.
- Inflammation or chronic illness — the body makes less transferrin and holds iron back in storage.
- Liver disease — transferrin is made in the liver.
- Low protein states — malnutrition, or kidney conditions that leak protein into urine.
Read it as a pattern
